Athletic Emergency Contact Form - Every athlete/parent must fill out annually.
SMC Co-Curricular Code Signature Form - Every athlete/parent must sign this form annually.
WIAA Physical Form - for Freshman and Juniors. Every athlete needs a physical every other year.
OR
WIAA Alternate Year Form for Sophomores and Seniors-use this form in years that you do not have a physical.
Athletic Transportation Form - fill out and hand in to the activities director or head coach if your son/daughter will not be using school provided transportation to and from SMC.
SMC Program Evaluation Form - to be filled out by parents, follow the directions on page one.
SMC Athlete Evaluation of Past Season Form - to be filled out by athletes, follow the directions on page one.
